Create a Survey for Myspace





Welcome to the Create-a-Survey Generator This is an easy way to create a survey to complete and post in your blog and have others do the same thing. Once completed you will be provided with the source code to post into your blog.

Instructions:
  • Enter one question per line
  • Select the "header/title" from the pull down menu to give a title to the following group of questions. (not required)
  • You will be able to enter your answers once you submit the questions.
  • Once you entered up to 100 questions hit "submit survey" at the bottom of the page
